A Decomposition Analysis of Factors that Increase Therapeutic Rehabilitation Service Expenditures

Abstract
Where are the current rehabilitation services going now and which factor is it contributing to? For this study, we used health insurance claims data (sample cohort DB, 2006~2015) to check the trends in the increase in prescribed therapeutic rehabilitation services and analyzed, by using the Log Mean Divisia Index, factors that contributed to changes in expenditure on therapeutic rehabilitation. We decomposed changes in expenditure on therapeutic rehabilitation services into demographic and medical use factors. After that, the contribution of detailed factors by treatment area was identified. Lastly, analysis has been performed by age group, types of medical service and medical care institution.
The increase in therapeutic rehabilitation expenditure in the past decade was attributed more to increased number of patients and increased amount of rehabilitative therapy per patient (76.8%) than to demographic changes (23.2%).
The contribution of physical therapy (79.5%) was much larger than that of non-physical therapy (20.5%). Professional therapy accounted for 57.9% of physical therapy. Those patients both less than 10 years old (4.5%) and over the age of 50 (81.9%) have been seen to contribute to the cost increase in professional physical therapy comparatively. From the characteristics of the service provider, hospitals (88.6%) or inpatient in long-term care hospitals (52.5%) has taken up most of the increase. In addition, it was founded that the use of various professional personnel is increasing along with the increase in demand for professional rehabilitation treatment (10.6%), and the costs of the pain treatment is increasing due to the aging (12.3%). Finally, it was suggested that the establishment of a rehabilitation medical delivery system and the improvement of the medical fee system are also necessary to provide integrated and specialized patient-centered rehabilitation services.
